I have not made it myself, but I am thinking there is two ways to make it at least.

One way would be to take each button and pass wire through the button holes to make loops in the back.  Then using those wire loops to string the buttons onto wire or chain to make the bib piece. 

The second is to just use wire and wrap the buttons along and shape the bib as you go along.

The first would make for a more flexible bib piece and keep it random and flexible.  The latter would give you a piece that would be more fixed, but keep a specific pattern or design in place, but may not be as flexible.  Also, it may be more prone to flexing, twisting, and wear on the wire.
