So the circumference of a hat is 21". It is hooked with a 6mm hook. Upon measuring my head, I measured 21 1/2" (22" comfortably wrapped around).

My questions are, should I make the circumference 22"? What hook should I use to get it there?

If there is stretch in the hat, then you wouldn't have to worry too much.  Other than that you may have to expirement with going up a hook size to get your gauge.
