This is my first post. I just found this website last week, and have been on it ever since then! You are all inspiring!!!
Here is a shirt I put a design on w/ a Sulky Pen... I was going for the faded-out.. not perfect look..

Wow, that looks really good, great job! I've been meaning to do an ink transfer on some of my clothes but have been putting it off in fear of messing up lol. Oh and I really like the faded look.

here you go...

step one: Purchase a Sulky Transfer Pen
Step two: print out a picture you like off the computer and reverse
step three: use sulky pen to draw over your picture
step four: place paper with the picture touching your shirt (must NOT be 100% cotton)
step five: iron according to the directions with the sulky pen

is the shirt 100% cotton?  that may be why its fading.  you need something with polysters in it.  i learned the hard way

its probably a Sulky pen.  you can get them at Michaels or Joanns...at michaels, they're 3.49 a piece i think.
