I had a little trouble with the patterns from that book as well. Fortunately the author has a blog where she's put up a tutorial on this exact thing - here you go:
There are tutorials on how to do the hair at this site as well.

Rather than try to sew a chain onto your crochet, why not just slip stitch directly onto the surface? Here's a page that gives instructions and has several examples of the technique I'm talking about.
