That's from the same company - I googled Charted Imagery and found a number of places that sell their kits.

Was it expensive?

I paid about $12 Canadian for the pattern.

Not at all because all the sides are connected to the bases.  You just pull the sides together and put the lid on.

Yes, that is the name of the company.  This is what I found for their contact information (their is an e-mail address link at the bottom)

craft_ee_me, that looks very similar to what I did only mine had more sides and the little box in the middle.

Added Nov. 29, 2007 - I was back at this same craft show and bought another kit and the designers told me they now have a website.  It's http://needleboxesetc.com/

In December I was at a large craft show and came across a booth with a product I had never seen before.  They offered either the completed project or the pattern to make one.  Being the crafty person I am (and cheap), I decided to buy the pattern and make it myself.

The lady who sold it to me said that it should take about 4 hours (but it took me way longer) and the only thing I needed to provide was the material and white craft glue.  There is no sewing involved in this, just gluing

This is called a six sided box and they had many more patterns - a light house, outhouse, bird house, dog house, 8 sided box, 4 sided box and ones they call grandma and grand-daughter boxes.  I think I'm going to be ordering more from them!
