WHy she write Cast on, then PM, then CO?

does she really mean Cast off?!
 

nope. one thing to note about patterns is that it's always CO (cast on) and BO (bind off). here you are casting on a few stitches, placing a marker for the center of the V neck, and then casting on a few more stitches. this will give you a wide V neck for the ribbing that you will do around the neck later.

did you do waist shaping as it would be similar to what i would have to do i think you said you did the 40" (which i am doing) and are using the same yarn...

I didn't have to do any type of extra shaping.  The neckline was lower than I expected but overall I'm pleased with the way it looks... I don't mind wearing a cami underneath.  Oh, I use color #08.
