I followed the iron on transfer instructions and also consulted craftster and read the tips here.  What I want to know is if anyone else has had this problem/what the solution for next time might be.  The transfer looked good after I spent 4 minutes ironing and i peeled off the paper backing when it was cooled.  I took the shirt, turned it inside out and washed it alone in cold water.  When i took it out of the washer, it was all crackled and peeling off.

So, before I spend more time and effort trying this again... what can I do differently?  I should mention that they were Avery brand transfers and I printed the transfer with my HP laserjet a few days ago and stashed it so the BF didnt see the present I was attempting to make.  Where did I go wrong?

The only things I can think of are (after checking the instructions with my package of iron-on paper):
You ironed at too high a temp.
You ironed it too long.
You washed it too soon after applying.
