A friend's grandmother gave me this and I have no idea what it is!

It might be an antique punch rug hooking tool.  Does it have a manufacturer's name on it?

It does look like a needle punch for either rug punching or punch embroidery..nice!

If it's large-ish I agree with the rug punch theories.  If it's smaller it might be a Russian punch needle.  Same idea, but used to make tapestries for bags, wall hangings and other upholstery projects.
