I'm trying to crochet a vest and the line in the pattern goes like this:

Row 4: Ch 1, sc in first dc, *skip next 3 dc, shell in next dc,
skip next 3 dc, sc in next dc; repeat from * across, turn
Row 5: Ch 4, skip next ch-1 space, [sc in next ch-1 space,
ch 1] 4 times, *skip next 2 ch-1 spaces, [sc in next ch-1
space, ch 1] 4 times; repeat from * across to last dc, dc in
last dc, turn

I'm a little confused about Row 5. Does it mean I only do sc in the ch-1 spaces, or do I treat each stitch as a ch-1?

From how I read it I would say you do a sc in each ch 1 space and skip the stitches in between them.  Give it a try like the pattern reads and if it seems really tight then I think there might be a typo because it says to ch 4 at the beginning but just a ch 1 between each ch1 space.  Does this pattern repeat anywhere else on the vest?  Check to see if those directions are the same.  Let me know if you need more help.

Or try [sc in next ch-1 space, ch 4] 4 times, that's where I think there may be a typo.
