I've got quite a few cute shirts that are too small in the neck and arms so I may have to have a go at this.

I'm assume you just cut along the top seam, sew ribbon to the edge, add grommets (always the part that's the most pain in the butt) and then lace it up?

Oh yea I cut the seam and then I sewed the grommet ribbon.
I got the grommet ribbon at Jo Anne's with a 40% off coupon. It's around 5.99 a yard I think.
I used t-shirt yarn for the lace part!
