So, I just signed up for my first OTT swap... maybe I shouldn't have done that given that I'm about to ask what exactly counts as Tiny... but I was excited by the theme!

So, I know that 'Tiny' refers to the physical size of the item, but is there anything else I should know?
Does it have to be a little mantelpiece trinket, or can it be something with more purpose (say, an embroidered needle book, or a beautiful decorated magnet, or a broach?)
How much work is typically required for a OTT - are there guidelines akin to the points systems on other swaps?

Did it say anything in the swap guidelines? Usually they don't leave it as just OTT (i.e. there would be examples...) ...

But my understanding is that most OTT swaps involve an item that is 4" or smaller in any given direction. The item itself can be anything you (or hopefully your partner) wants it to be, as long as it fits that size requirement. Time investment can vary, but you would want it to look like something you spent a decent amount of time/ effort on.

And yes, it can be functional - jewelry, drink coasters, needle case, small art piece.

Your OTT can be anything (decorative or useful or both) as long as it fits a 4x4x4" dimension.

For a OTT, you should craft it to the best of your abilities and really stretch those craft muscles.  Make something that you can craft with a high level of skill.  Even if it is a fairly simple/easy item for you to make, you can embellish so that it best represents your skill.  (For example, if you wish to make a coin purse, you could embroider, add beading, or take other steps to make it a really special piece.

I would say the OTTs I have seen are mostly mediums aka 2pt., but I have seen a fair share of larges aka 3pt.  Smalls aka 1pt. would not really fit into the spirit of crafting a OTT.

I suggest checking out OTT swap galleries to really get the feel of OTTs.  A few fab galleries....
