I've seen several blouse patterns on Simplicity or Butterick that are very close to what you want.

It looks like a straight, standing collar with ruffled detail and a sort of cravat tied over top (that's the big ruffle in the front of the throat with the pin). The shirt sleeves are also just a normal, straight sleeve with two, cascading bells attached to the bottom. It's hard to explain xD I'm half asleep. I tried to draw a quick pic to explain, but that ended up just as discombobulated.
