How can I make this?

I want to make something similar to this for my boyfriend for our aniversary:

Those look awesome. And go with the Lapis or Pietersite for a stone. Sophisticated and masculine.

They are awesome, and not too difficult to make.  I'm going with some green labradorite stones, but I may make a whole stack of them for my boyfriend!
