Swap Organizer himself/herself meets these conditions:
- Has fully read the Swap Info Guide: YES/NO
- Has successfully completed two swaps as a participant: YES/NO
- Is not organizing more than three swaps right now: YES/NO
- Will recruit a co-organizer or give all details to a Swap Moderator if this swap has more than 25 participants: YES/NO
- Is at least 18 years old: YES/NO

Swap Organizer will check each participant to make sure they meet these conditions:
- Has been a member for at least one month: YES/NO
- Has posted at least 15 times: YES/NO
- Has completed one swap successfully before signing up for multiple swaps at a time: YES/NO
- Is not currently signed up for more than five swaps: YES/NO
- Does not have any negative feedback: YES/NO
- Participant has agreed that he/she is at least 16 years old: YES/NO
- If yes, understands that Delivery Confirmation number is required for this swap: YES/NO
- If no, understands that a mailing receipt copy is required for this swap: YES/NO

Details of swap:
     Do you love poetry? Do you write poetry, perhaps? Or do you just read it, maybe memorize it; devour an entire volume in a night or sip slowly at the words over days? Are you inspired by the imagery, moved by the beat, or is it the message behind the words that sparks your imagination? Do you just think it would be fun to craft around a poem or three? If the answer to any of this is yes, then this is the swap for you!
     We will be crafting one medium item and two small items (thats 4 points total, if you use the points system) inspired by our partners favorites. Please send your partner at least 3 poems total by at least 2 different poets.
     Communication is important! Please be sure to keep in touch with your partner; the more you get to know them, the more fun it is to make things for them. You also need to either post on the board (or PM the organizer if youre not chatty  Smiley ) at least once a week.

Small Item = 1 point
Med Item = 2 Points
Large Item = 3 Points
 
For new swappers there might be a bit of a confusion as to what is a large, medium, and small item, so here's a handy guide:  

Small = By time, 2 or less hours of crafting. By $$$, $10 or less. By size, Earrings, stickers, wallets, pouches, bookmarks

Medium = By time, 2 or more hours of crafting. By $$$, $10-$30. By size, one single necklace or bracelet, a scarf

Large = By time, 3 or more hours of crafting. By $$$, $30 or more. By size, A shawl, a shirt, a skirt, a set of jewelry with four to five items, a purse
 
This is not totally set in stone, but is a rough estimate of sizes described by time it takes to make package, by physical size and by cost.

Are there any items/ themes/ subjects that you would NOT like to receive?
Do you have any preferences as to fonts and handwritten v/s printed words?
Would you be okay if there were no words in your finished items, but only images inspired by your poems?
Do you have any allergies?
Do you have any allergens in your home?
Anything else you want your partner to know?
Anything you want the organizer to know or take into consideration for partnering you? (This will be omitted from the questionnaire that your partner receives)

Gah.  I think this is one of the hardest forms to fill out!  I have so many favorites but now that I'm trying to list some I keep forgetting what the titles are or only remember one line.  Now I'm at a standstill because I forgot I had a notebook with favorite poems/quotes and it's at home.  I may have to pm my partner later as I remember more poetry.  I also think I'm thinking too hard on this one.
