Sometimes the organiser will scout around for just that one more person.
As an organiser I've once partnered myself with two people (ie - sent out two parcels and received two in return) just to avoid problems.

But usually, a three-way chain is devised, where X sends to Y who sends to Z who sends to X.

Yep, Kitten Wrangler is right! If there ends up being an odd number of participants, I plan on doing a 3 person chain. And, you actually have just made this even, because I realized that I forgot to add someone to the front list. So now we're at 28!
