My first thoughts are HEH? I'm really confused and would like a little bit of help thanx!

So you have 21 stitches on a holder.  You are going to cast on 30 with a new strand of yarn, and then, continuing with the same strand that you just used to cast on, you will knit the 21 one stitches off of the holder, then cast on 30 at the end of the row.  You will have 81 stitches, with bottom part you have already completed in the middle.

I hope this helps!!
