hmm. i'm not sure exactly what you mean, because i never have that problem.
what you need to do is make sure that the shirt is flat at all times.
around the armpits you just need to make a smooth curve (keeping the shirt flat!) then it should turn out fine.
does that help?

Adorable!  I keep meaning to do this with tons of my own shirts, but my sewing confidence is low.  I'll keep my fingers crossed that I come out with something this nice.
