I wish I could help.  I think (and this is a serious think.  I did this a few months ago and I dont' have my book) that it has something to do with reserving the very center 18 stitches as the neck portion.  Like, if you make a small it will have less stitches across (in the previous row) than if you were to make a large, so it can't give you an exact number to crochet.

So I belive you take the number of stitches that you have done in the previous row, subtract 18, then divide by two.  Then you use that number to make the front part.  So if you have 40 stitches on the previous row, you subtract 18 which makes 22.  Then you divide 22 by 2 which is 11.  So you would crochet 11, then turn for the next row.  Make sense?

I hope this is right!  That's how I seem to remember it.  Someone please correct me if I'm totally off.

Oops!  Sorry, I was probably one of those 17 people - I've made cupcake but didn't have the book to hand - I really did mean to come back!

JanxX is right - you just need to work out where the central 18 stitches (for the back of neck hole) are and use stitch markers of some kind (bits of thread, bobby pins, paper clips or whatever) to mark the first and last of them.  You then work the shoulder part of the back on either side with the remaining stitches (exact number will depend on the size you're making).  Good luck!
