I added the basic/average times constraints. But small would be maybe a couple of earrings and some button/pins. Maybe a small set  of ATC's A large (which is not required but encouraged) could be a stenciled shirt or knitted/crocheted scarf (as they take a few hours) and a medium could be  a set of decorated notebooks or something. Quite honestly, talk with your partner and agree on it, much easier.

I would agree. It can very much vary - for the last Disney swap I did, my partner and I agreed the "small" is the one you spent the least amount of effort on.

Here's a case study because that's not very helpful. And I understand fully the confusion in trying to understand these things!

I did Disney Swap Round 4, and my partner and I did 1 large, 1 medium, 1 small.

Large: A knitted purse with photo pockets (my partner did a shaped and crocheted scarf that I absolutely adore!)
Medium: Predone armwarmers that I jazzed up with some embroidered felt pieces and handwoven ribbon (my partner did a decorated journal with hand altered pages).
Small: Bottlecap earrings and chain necklace (my partner included a cute hat and tin from Disneyworld since I had never been there before)

Does that help? I think the biggest idea is all in how much time you take to craft the item - but you do have to talk to your partner a whole bunch!
