I got this kwik sew pattern -- http://www.thesewingplace.com/index.asp?PageAction=VIEWPROD&ProdID=2001, which is a nice basic bias top and wrap skirt.  But I'm making four of these shirts and wanted to change them up a little, so I added neck line pleats...

Here are the pleats themselves, that I interfaced, sewed and ironed the hell out of.  I sewed this on to the shirt as a big rectangle on the last step of the bias binding neckline topstitching (for a bias binding neckline tutorial, see this link -- http://www.diynetwork.com/diy/na_sewing/article/0,2025,DIY_14144_2276785,00.html

here's the inside so you can see how it looks once its trimmed.  I also zigzagged the edges so they don't fray.

The front, though it lays flatter than that in real life - I'm just holding the camera weird.

I added pleats to the back too

the shirt itself was really easy.  The pleats took a little while, but I think it'll be fun to wear!  I stiff thick pleats like these would look nice on a knit shirt too, but I haven't tried it out yet.
