The exact same thing happened to me - I am the oldest on both sides of my family and by the time I was in college, we started the name drawing, so I made a ton of stuff for my cousins! 
One year I made everyone jester hats out of fleece - super easy, you need just a few carnival colors of fleece and some (optional) jingle balls.  Another year, I knitted easy beanies - with school colors or favorite pro sports team colors.  You can also make frilly flip flops with novelty yarn and a crochet hook, crocheting around the foot straps (those were a huge hit for me).  And finally, with little bitty kids, you can never go wrong with simple stuffed animals...
