I made a hat once for my cat, Charlotte. I used a pattern from a great book called "The Doll's Dressmaker" by Venus Dodge. The patterns can be traced right from the book and can be resized to fit a puppy or kitty head. Charlotte dies right before xmas (she was 14 years old), but I still had this cute pic of her in her hat (which she hated wearing, btw!):
