I know this isn't the most unique costume idea, but in my defense, my daughter's name is Alice, and she happens to have blond hair and blue eyes. I based her costume on the Disney movie (obviously). I made the dress from New Look pattern 6448, with a few modifications: I added a waistband on the dress (the original was to empire waisted), I added 2 layers of tuille between the slip and outer skirt to poof it up, and I added a ruffle on each skirt to look like petticoats. I made the headband out of scrap black cotton I had on hand, and we bought the shoes, tights & turtleneck (its cold here on Halloween). I made my own pattern for the apron. Both the dress and the apron were made from old sheets I already had, so the dress could've been a little blue-er, but I think it still looks good. She won a costume contest for her age group at our VERY small town's Halloween festival and got a bike! Its way too big for her (she's 3 1/2 and can barely pedal her trike), but none the less, she loves her new "bikey".
Of course, I didn't take any "making of" pics, but oh well- it was your standard lined dress.
