Have you seen spoonflower.com? they let you upload images and print on fabric. There can be some colorshifting, but I did a nice picture of a hummingbird and it came out really nicely. I just got it in the  8" sample size.

I use zazzle.com they look really great and they have lots of shirt choices.

I think getting it perfectly flat so that it would stamp evenly would be a problem.
