These are pictures of a washcloth from a towel set that was a wedding present. 

My husband's great aunt crocheted the edging.  I want to copy this style onto a baby blanket.  My trouble is that no instructions I can find cover how to attach the trim.  They all assume you are going to sew it on after you made it.  I know these pictures are a little fuzzy, but the trim on these towels was clearly crocheted directly onto the towel.  How do I do that?  Just keep a needle threaded ahead of the hook?  Obviously I would only need it for the first round to do the foundation chain.  Should I use a tiny hook instead and hope I can force it through the fabric?

I'm going to start my first wearables project, and I've already run into a snag.  How do I adjust for size?  This pattern has 5 options for size, which is super helpful; except my friend is between them!  She falls into the finished sizes for the waist L and hips XL.  Do I just make the whole skirt XL and hope she can simply gather more at the top with the drawstring?  Or do you have suggestions for how to go about increasing my increases even further? 
Here are the numbers just in case it helps.
Friend: waist 36", hips 48" 
Pattern: L waist 36", hips 44"    XL waist 40", hips 48"
I think I can accomplish either method, I just want to know what you lovely crafters think would work best.  Also, if I do change the increases...How much?  When?  HOW?
